Carefully complete your selection of 5 Director Candidates, print your name and sign it. Then mail it to the address shown on the ballot on or before December 15th.

Ballots with no member name will be discarded. Only members may vote.

Each member may only submit one ballot; only one vote per member. Duplicate or multiple ballots from the same member will void all of them. If you submit an electronic ballot via the email, do not mail a printable ballot. If you send a printable ballot, do not complete the electronic ballot via email.
